How to complete a form
- Choose the form that matches your program or the club’s instructions.
- Have the player’s details and your correct email ready. A parent or guardian should complete forms for a child.
- Read the information, check your answers and any fee, then submit once.
- Keep the confirmation or reference. If you are unsure whether a payment went through, contact us before paying again.

Summer Goalkeeper Programme
Build goalkeeper skills and confidence with Dully at Polly Parade Reserve, Tarneit.
Saturdays, 8:30–9:30 am.
Please arrive at 8:15 am.
$120 for the eight-week programme. Optional Get Active Kids voucher, subject to club verification.

Use the correct player name and keep your order or registration reference. Submitting a form does not by itself confirm a bank transfer or voucher payment.
$70 · Home & away · Allow 30 days
Uniform order — home & away kit
Order one home shirt and shorts, plus one away shirt and shorts. Socks are not included.
Complete kit: $70.
Please allow 30 days for delivery after submitting the form.
How to fill it: Enter the player’s age, height and requested sizes. Check sizing against a similar-sized player’s kit, complete payment and read the collection instructions.

Game-day volunteer support
Help match days run smoothly for players, coaches and families. Support may include setting up and packing away equipment, welcoming families or assisting with team organisation.
You can offer the time you have available. TNFC will confirm your task, arrival time and any role requirements before the match.
